Apply for Evaluation

 

Assisted living facilities, skilled nursing facilities, continuum of care residential communities, and adult day care/adult day healthcare programs throughout the United States may apply to participate in the Excellence in Care Program of Distinction. Excellence in Care Specialists are available to conduct on-site evaluations across the nation.
